Summary and Comment: The autopsy report states:

Cause of death: Inhalation of smoke and carbon monoxide due to fire; extensive charring.
This subject emanated from commingled remains formerly designated as 64-68 and was subsequently designated MC Doe 67-1, B2 after anthropologic sorting/evaluation.

II. The FBI's October 8, 1993 Report to the Deputy Attorney General on the Events at Waco, Texas (cached)

Summary and Comment: The DOJ Report states:

Unidentified Branch Davidians From April 19 Fire … Doe 67-1, a 5-6 year old girl, died of smoke and carbon monoxide inhalation. Her body was found in the bunker.
